Below are requirements to work at All Star Home Health Care.
- A PCA provides care as stated in the client care plan and in accordance with Department of Human Resources (DHS) standards.
- Review our PCA Job Description to see if you meet the qualifications.
- Prior to applying for a PCA position, you must complete the DHS online training course.
- Click on this link to the Minnesota Department of Human Services Individual Personal Care Assistant Training to complete the test.
- Once you pass the test, print several copies of your certificate. Include a copy of your certificate to All Star when you submit an application.
- Please fill out all these required forms: CFSS Worker Enrollment Application, BGS Form, Federal W4 Form, Federal I9 Form, Provider agreement form
